The Role of Social Support in Moderating the Relationship between Race and Hypertension in a Low-Income, Urban, Racially Integrated Community

In the US, African Americans have a higher prevalence of hypertension than Whites. Previous studies show that social support contributes to the racial differences in hypertension but are limited in accounting for the social and environmental effects of racial residential segregation. We examined whether the association between race and hypertension varies by the level of social support among African Americans and Whites living in similar social and environmental conditions, specifically an urban, low-income, racially integrated community. Using data from the Exploring Health Disparities in Integrated Communities-Southwest Baltimore (EHDIC-SWB) sample, we hypothesized that social support moderates the relationship between race and hypertension and the racial difference in hypertension is smaller as the level of social support increases. Hypertension was defined as having systolic blood pressure greater than 140 mmHg and/or diastolic blood pressure greater than 90 mmHg, or the participant reports of taking antihypertensive medication(s). The study only included participants that self-reported as “Black/African American” or “White.” Social support was measured as functional social support and marital status. After adjusting for demographics and health-related characteristics, we found no interaction between social support and race (DUFSS score, prevalence ratio 1.00; 95% confidence interval 0.99, 1.01; marital status, prevalence ratio 1.02; 95% confidence interval 0.86, 1.21); thus the hypothesis was not supported. A plausible explanation is that the buffering factor of social support cannot overcome the social and environmental conditions which the participants live in. Further, these findings emphasize social and environmental conditions of participants in EHDIC-SWB may equally impact race and hypertension.

Racial non-equivalence of Socioeconomic Status and Health among African Americans and Whites

Racial health inequities are not fully explained by socioeconomic status (SES) measures like education, income and wealth. The largest inequities are observed among African American and white college graduates suggesting that African Americans do not receive the same health benefits of education. African Americans do not receive the same income and wealth returns of college education as their white counterparts indicating a racial non-equivalence of SES that may affect health inequities. The aim of this study is to determine whether racial non-equivalence of SES mediates race inequities in self-rated health by education and sex. Using data from the 2007–2016 National Health and Nutrition Examination Survey in the United States, the mediation of the associations between race and self-rated health through household income ≥400% federal poverty line, homeownership, and investment income were assessed among college graduates and non-college graduates by sex. Indirect associations were observed among college graduate women (odds = 0.08, standard error (s.e.) = 0.03), and non-college graduate men (odds = 0.14, s.e. = 0.02) and women (odds = 0.06, s.e. = 0.02). Direct associations between race and self-rated health remained after accounting for household income and wealth indicators suggesting that race differences in income and wealth partially mediate racial inequities in self-rated health. This study demonstrates that the racial non-equivalence of SES has implications for health inequities, but the magnitude of indirect associations varied by sex. Other factors like discrimination, health pessimism and segregation should be considered in light of the racial non-equivalence of SES and racial inequities in self-rated health.

The Endogeneity of Race: Black Racial Identification and Men’s Earnings in Mexico

A growing body of sociological research has shown that racial identification is not only fluid, but crucially depends on other individual- and societal-level factors. When such factors are also associated with socioeconomic outcomes such as earnings, estimates of the disadvantage experienced by individuals because of how they identify racially obtained from standard regression models may be biased. We illustrate this potential bias using data from a large-scale survey conducted by the Mexican census bureau. This survey is the first by the government agency since the country’s independence to include a question on black identification. We find evidence of a substantial bias in estimates of racial disadvantage. Results from our initial models treating racial self-identification as an exogenous predictor indicate that black men have higher earnings than non-black men. However, when we use an instrumental variables model that treats racial self-identification as endogenous, that is, as a function of the same unobserved characteristics as individuals’ earnings, we find a significant negative effect of black identification on earnings. While previous studies have acknowledged the endogeneity of race, ours is the first to explicitly model racial self-identification as an endogenous predictor to obtain an unbiased estimate of its effect on individuals’ socioeconomic conditions.

Race-Ethnicity, Class, and Unemployment Dynamics: Do Macroeconomic Shifts Alter Existing Disadvantages?

Research indicates that individuals of different races, ethnic backgrounds, and class origins differ in their unemployment rates. We know less, however, about whether these differences result from the differing groups’ unequal hazards of entering or exiting unemployment and even less about how economic fluctuations moderate the ethnoracial and class-origin gaps in the long-term risks of transitioning into and out of unemployment. Using Rounds 1–17 of the National Longitudinal Survey of Youth 1997 and event history models, we show that non-Hispanic blacks become more similar to non-Hispanic whites in their paces of entering unemployment as their local unemployment rate rises, perhaps because jobs largely closed to the former are eliminated in a greater proportion during recessions. Nonetheless, blacks’ relatively slow pace of transitioning from unemployment to having a job decelerates further with economic downturns. By contrast, Hispanics’ paces of entering and exiting unemployment relative to non-Hispanic whites hardly change with local unemployment rates, despite unemployed Hispanics’ slower rate of transitioning to having a job. With respect to class origin, we find that the advantages in both unemployment entry and recovery of young men with relatively educated parents diminish with economic deterioration. Based on these results, we suggest that facing economic pressure, employers’ preference for workers of a higher class origin is more malleable than their preference for whites over blacks, making unemployed blacks especially disadvantaged in uncertain economic times. Blatant, Subtle and Insidious: URM Faculty Perceptions of Discriminatory Practices in Predominantly White Institutions

Although modest gains are observed in the number of African American, Mexican American, and Puerto Rican faculty in higher education institutions, systemic issues of underrepresentation and retention remain problematic. This article describes how historically underrepresented minority (URM) faculty in Predominantly White Institutions perceive discrimination and illustrates the ways in which discriminatory institutional practices—such as microaggressions—manifest and contribute to unwelcoming institutional climates and workplace stress. Using a mixed methods approach, including survey data and individual and group interviews, findings show that respondents ( n = 543) encounter racial discrimination from colleagues and administrators; experience discrimination differently based on their race/ethnicity and gender; and report difficulties in describing racist encounters. Qualitative data reveal three themes that inform the survey results on perceived discrimination: (1) blatant, outright, subtle, and insidious racism; (2) devaluation of scholarly contributions, merit, and skillset by colleagues and administrators; and (3) the burden of “representing minorities,” or a “racial/ethnic tax.” Propositions for how to change unwelcoming environments and create safe spaces for professional development to reduce the adverse effects of discrimination among URM faculty are discussed.

"How Do You Advance Here? How do You Survive?" An Exploration of Under-Represented Minority Faculty Perceptions of Mentoring Modalities

This article contrasts perceptions among 58 under-represented minority (URM) faculty employed at U.S. research-extensive universities who reported an absence of mentoring or experienced informal or formal mentoring modalities. Key findings reveal a mentoring glass ceiling that affects URM faculty career paths: an absence of mentoring can lead to significant career miscalculations; well-intentioned mentors can devalue faculty scholarship; lack of senior faculty accountability for observed disengagement from faculty career development; and inadequate mentorship often limits access to social networks and collaborative research opportunities. Recommendations are offered for developing effective formal mentoring initiatives that reflect an institutional investment in early-career URM faculty.

Collaborative Science and Learning as Tools for Climate Change Adaptation Planning

Anticipated impacts from climate change act as stressors that motivate adaptation strategy development. And, while climate science projections extend from the global to regional scale, they can leave significant uncertainty at the local scale. In many jurisdictions, governance and environmental management professionals formulate and distribute information to guide climate change policy and preparation. In many rural or otherwise marginalized areas, however, relationships needed to promote clear understanding of impacts and to tackle cooperative adaptation planning alongside residents are lacking. This article discusses methods used by an interdisciplinary group of scientists to help a small community of rural coastal United States residents enhance their climate resilience. This was accomplished via participatory collaborative science and collaborative learning processes that facilitated relationships of trust among a broad group of stakeholders. Data gathered from our network and analyses of project activities show the benefits of collaboration across a social network representing the social-ecological system. The success of our efforts is evident in five ways: a) in localized application of climate and environmental knowledge, b) in building two-way knowledge across the local/nonlocal divide, c) in incorporating local community values, d) developing trust between residents, scientists, and environmental governance and management professionals, and e) in lessons learned transitioning from a learning to decision-making process. We strongly advocate those working with local groups on adaptation planning efforts begin with methods that help build knowledge, respect, trust, and capacity among residents.

Enhancing socio-ecological resilience in coastal regions through collaborative science, knowledge exchange and social networks: a case study of the Deal Island Peninsula, USA

Collaborative science brings together diverse stakeholders to share knowledge and form networks that in turn can be foundational to policies and practices to increase socio-ecological resilience. In this article, we present results from a collaborative science project that employed collaborative learning methods to develop a network of local, regional, state and academic stakeholders. These stakeholders had little social interaction prior to the project and represented a diversity of views, positions and responsibilities. They shared in common a concern for the effects of climate change on a coastal socio-ecological system and the desire to reduce vulnerabilities and enhance resilience. Through ethnographic and survey methods, we found that collaborative science and learning promoted the exchange of cultural and environmental knowledge and expertise among individuals who previously had no sustained interaction. Stakeholders perceived these exchanges as worthwhile in that they allowed individuals to express viewpoints and share knowledge and expertise, which was seen to have the potential to increase socio-ecological resilience. Our results suggest that social networks can emerge from collaborative science and learning projects and can become formally organized and help foster opportunities to enhance socio-ecological resilience.

Maternal experiences of ethnic discrimination and child cardiometabolic outcomes in the Study of Latino (SOL) Youth

Purpose Limited research has examined maternal experiences of racial/ethnic discrimination in relation to child cardiometabolic health. In this study, we investigated whether maternal experiences of ethnic discrimination were associated with cardiometabolic risk in Hispanic/Latino youth several years later. Methods Our sample included 1146 youth (8–16 years) from the Study of Latino Youth (2012–2014), who were children of the Hispanic Community Health Study/Study of Latinos participants (2008–2011). We used regression models to examine the prospective associations between maternal report of ethnic discrimination in relation to her child's body mass index (BMI) z-score, metabolic syndrome score (MetS), and high sensitivity C-reactive protein (hsCRP) levels 2 years later. Results Maternal ethnic discrimination was associated with youth hsCRP, but not BMI or MetS (P -values >.05). Adjusting for age, nativity, and national background, maternal ethnic discrimination was associated with higher (log) hsCRP levels (β = 0.18, 95% CI = 0.04 to 0.32) in children. This association was robust to adjustment for maternal and household characteristics (β = 0.17, 95% CI = 0.04 to 0.31), as well as maternal depression and maternal BMI. Conclusions Maternal ethnic discrimination is associated with inflammation among Hispanic/Latino youth, and not BMI z-score or MetS. Studies are needed to address temporality and pathways.

Association of Childhood Asthma With Federal Rental Assistance

Importance Millions of low-income children in the United States reside in substandard or unaffordable housing. Relieving these burdens may be associated with changes in asthma outcomes. Objectives To examine whether participation in the US Department of Housing and Urban Development’s (HUD) rental assistance programs is associated with childhood asthma outcomes and to examine whether associations varied by program type (public housing, multifamily housing, or housing choice vouchers). Design, Setting, and Participants This survey study used data from the nationally representative National Health Interview Survey linked to administrative housing assistance records from January 1, 1999, to December 31, 2014. A total of 2992 children aged 0 to 17 years who were currently receiving rental assistance or would enter a rental assistance program within 2 years of survey interview were included. Data analysis was performed from January 15, 2018, to August 31, 2019. Exposures Participation in rental assistance provided by HUD. Main Outcomes and Measures Ever been diagnosed with asthma, 12-month history of asthma attack, and 12-month history of visiting an emergency department for the treatment of asthma among program participants vs those waiting to enter a program. Overall participation was examined, and participation in public or multifamily housing was compared with participation in housing choice vouchers. Results This study included 2992 children who were currently participating in a HUD program or would enter a program within 2 years. Among children with an asthma attack in the past year, participation in a rental assistance program was associated with a reduced use of emergency departments for asthma of 18.2 percentage points (95% CI, −29.7 to −6.6 percentage points). Associations were only found after entrance into a program, suggesting that they were not confounded by time-varying factors. Statistically significant results were found for participation in public or multifamily housing (percentage point change, −36.6; 95% CI, −54.8 to −18.4) but not housing choice vouchers (percentage point change, −7.2; 95% CI, −24.6 to 10.3). No statistically significant evidence of changes in asthma attacks was found (percentage point change, −2.7; 95% CI, −12.3 to 7.0 percentage points). Results for asthma diagnosis were smaller and only significant at the 10% level (−4.3; 95% CI, −8.8 to 0.2 percentage points). Conclusions and Relevance Among children with a recent asthma attack, rental assistance was associated with less emergency department use. These results may have important implications for the well-being of low-income families and health care system costs.
